To register you need to complete BOTH FORMS, the GMS1 can be done online or by printing off and handing into the surgery on your first visit or email the forms to info.caritas@nhs.net

Return to the surgery with a valid ID - driving license, passport, bill form the last 3 months.

You will be offered a new patient health check with the healthcare assistant and we recommend that you take up this offer if a clinician has not seen you for some time.

If you have repeat medication, an appointment will be made for you to see one of the clinicians to enable continuation of your prescription.

Changes to Personal Details

Please inform us about any changes of name, address or telephone number in order that your records can be updated. The receptionist will be able to advise you whether your new address is within the practice area and will offer advice should you need to register with another practice.

To download our Change of Details form please the link below, complete it, and return to the practice either in person, by e-mail to info.caritas@nhs.net, or by post. Please remember to include the names of ALL the patients whose details you are changing. For example if you are updating your address and you have children who are registered with us, please remember to include their names and date of birth on the form so we can update their addresses too.
